DO NOT use the
Rotary International Application!
You are required to use
the 2009 Central States Outbound Student Long-Term Program Application, (August
2009 version). We will not accept any other application and/or part of
any other application.
This application MUST be
computer generated and/or typed.
No handwritten
applications will be accepted and all documents such as birth certificates,
transcripts etc must be translated into English.
We will only accept
completed applications which means you must submit all the additional
documentation required with your application. This will include copies of
your birth certificate, the past two years of school grades and/or transcripts,
a language proficiency letter for any secondary language coarse you have
completed by August 2009 such as Spanish, French etc if applicable.
The Country Selection
Worksheet with Individual Country Notes was designed for you to have a better
understanding of additional requirements that each country may have. For
example, some countries will require you to purchase additional health
insurance and possible a General Liability Insurance Policy at your expense
even though Central States will provide you with both of these types of
policies upon departure. Some countries may require you to attend a
language camp that you must pay for, some countries may require you to provide
a higher emergency fund than other countries. Some countries may require
you to have two years of German, or French, or they may have specific age
requirements for both younger and older students. We have tried to
outline the additional requirements that each country requires of you so you
will be able to make a better informed choice. However, country
requirements change all the time and this should only be construed as our best
effort to inform you of any requirements that we are aware of as of October 23,
